Some 18 months after opening, HMP Millsike continues to have hundreds of empty cells with inspectors saying it is unsafe to ramp up capacity further.

Mitie, which runs HMP Millsike, says the inspection report is not "a fair or accurate reflection".

HMP Millsike, on the edge of the East Riding of Yorkshire, opened in March last year to a cost of £400m.
